Better Health. Better ME!
Good communication between patients and their primary care providers is the best way to improve patient health, increase quality of care and reduce healthcare costs. To get this message out, M4A and Maineâ€™s AAAs and their partners have developed Better Health. Better ME!
The message is simple: An informed, engaged patient is at the heart of better health - for the patient, the community and our economy.
Better Health. Better ME! encourages older Mainers to talk with their primary care provider and gives them tools, like tips for asking questions, that will help them become more active participants in managing their own health. Primary care providers are also being asked to talk to their patients and have been given tools to help.
The advice from Better Health. Better ME! is just four steps:
1) VISIT your primary care provider.
2) ASK questions. Write down and ask questions so you understand your conditions and treatments.
3) KNOW your health care numbers (blood pressure, cholesterol, blood sugar, weight) and understand the steps you need to take to get healthier.
4) FIND and use community health resources and programs to help you get healthy outside the PCP visit.
